How it works

It is a simple but powerful tool. After identifying the problem, the team continuously asks “Why?” for each answer until the root cause is uncovered before applying corrective actions. Though five iterations are common, the process can take more or fewer steps depending on the complexity of the issue.

1 Why did the worker trip?

A pallet was left in the walkway.

2 Why was the pallet there?

It was used for restocking but wasn't moved afterward.

3 Why wasn't it moved?

The designed storage area was blocked.

4 Why was the storage area blocked?

Excess deliveries fom suppliers weren't stored properly.

5 Why weren't they stored properly?

There's no system in place for managing overflow storage.

Reasoning Behind

The 5 Why Tool uses a combination of reasoning methods to uncover root causes:

1. Deductive Reasoning: Narrowing down the root cause by asking targeted questions about the known problem.

2. Inductive Reasoning: General conclusions drawn from specific observations during the problem-solving process.

3. Abductive Reasoning: Forming the most likely explanation from incomplete information when full details are not available.
